TERMS AND CONDITIONS OF SERVICE
1. Nature of Services
RevivalRecovery provides peer recovery support, structured sober living housing, life skills training,workforce readiness services, and faith-based mentorship. RevivalRecovery is not a licensed medical provider, hospital, detoxification center, or psychiatric facility. Services are non-clinical unless otherwise specified in writing. Participants are responsible for maintaining independent medical, mental health, and medication management providers.
2. Colorado Legal Compliance
RevivalRecovery operates in compliance with applicable Colorado state laws, including but not limited to Colorado landlord-tenant law (C.R.S. § 38-12), mandatory reporting laws, and applicable Medicaid regulations where services are billed. Nothing in this agreement creates a medical provider-patient relationship. Housing participation may constitute a structured program agreement and may not create a traditional landlord-tenant relationship where services are bundled with residency.
3. Voluntary Participation
Participation is voluntary unless mandated by court order, probation, parole, or referral agency.
RevivalRecovery reserves the right to discharge participants for safety concerns, substance use,violence, illegal activity, non-payment (if applicable), or policy violations.
4. Housing Terms & Liability Limitations
• Participants reside at their own risk and assume responsibility for personal conduct.
• RevivalRecovery is not responsible for theft, loss, or damage of personal property.
• Participants agree to maintain renter’s insurance if recommended.
• Any injury resulting from rule violations, intoxication, or unsafe behavior is the responsibility of
the participant.
• RevivalRecovery is not liable for actions of other residents or third parties.
• Immediate discharge may occur for possession or use of drugs, alcohol, weapons, or violent conduct.
• Housing is contingent upon program compliance and may be terminated in accordance with Colorado law and program policy.
5. Faith-Based Environment
RevivalRecovery operates from a Christian foundation. Participation in faith-based components is voluntary unless explicitly stated in a specific program agreement. Services are provided without unlawful discrimination. Individuals are welcomed regardless of religious belief.
6. Confidentiality & HIPAA
Client information is protected consistent with HIPAA and applicable Colorado privacy laws. Information may be disclosed with written authorization, by court order, for mandatory reporting, or where there is risk of harm to self or others.
7. Dispute Resolution
Any disputes arising from participation shall first be addressed through the organization's formal grievance procedure. If unresolved, disputes shall be governed by the laws of the State of Colorado. Venue shall be El Paso County, Colorado.
